Credit Application
– are you a new customer? Have you not purchased from us in over 1 year? Please
complete the required credit application and return to us as soon as possible.
Once you are approved, you are free to place your orders.
Credit Card Authorization
– would you prefer to prepay for your order via a credit card? Complete this
authorization for quick processing.
Choose the form that represents the plant that you order from. Populate the
form and fax back to the client services phone number at the top of the page.
